Pharmacists play a very important role in the care team. We can also do a foot exam, glucose testing and refill their medication.
Some of those ways include connecting with the pharmacist to go over any medication issues they have, helping them navigate through their primary care physician, helping them with any DME products and if they have anything specifically clinical, that’s when I would pass them over to a clinician. There’s are so many ways we help patients with their diabetes. And we work together to provide the care that these patients need to help manage their conditions. The members have access to an entire team. Transform Diabetes Care is different than any other program. And I’m grateful for the position that I have now ‘cause I’m able to do just that. I wanted to be in the nursing field and help as many families as possible. And it took not only a physical toll on her, but it took a mental toll on her. Unfortunately, we didn’t discover that until she had her first heart attack.
